Coverage Change to Proton Pump Inhibitor Class Medications Effective March 19, 2007 a step-therapy requirement was applied to Protonix, Nexium, Prevacid, Aciphex and Zegerid. Step therapy means that certain medications must be tried in a set order for FCC to cover them. Read more.
Pharmacy Formulary The pharmacy formulary is available online. The formulary is effective as of January 1, 2007.
Medications Requiring Prior Authorization or Step Therapy FCC uses certain tools to help promote the use of safe and cost-effective medications and to reduce the potential for abuse or misuse of medications. These restrictions include prior authorization and step therapy edits on certain medications.
