PPO Plans
If the HealthSmart logo is listed anywhere on your health insurance card, your provider network is HealthSmart. To view the HealthSmart network directory, please click on the Health Smart logo below.	If the Medcost logo is listed anywhere on your health insurance card, your provider network is MedCost. To view the Medcost network directory, please click on the MedCost logo below.
All Other Plans
If the MedCost or HealthSmart logo is NOT listed on either the front or back of your card, your provider network is FirstCarolinaCare Insurance Company. Click on either logo below to view the FirstCarolinaCare Insurance Company provider directory.
